By signing his form you agree that keeping copies of any records you receive from this free vaccine clinic are your responsibility to maintain. You acknowledge that if you (or any Animal Control Office or any Veterinary Clinic) needs to contact the Little Guild for copies of the records that you will be responsible for a $25 administration fee payable to the Little Guild prior to copies being provided.
