DOG PLACEMENT REQUEST
OWNER SURRENDER
Surrender of an animal is a difficult and emotional decision. Please take the time to think through the finality of giving up your animal. If your animal is accepted for placement, all legal rights of ownership are relinquished at the time of surrender. Any person who surrenders an animal and then later changes their mind must fill out an adoption application to be considered as any other applicant would be considered.
The placement request form must be filled out in it's entirety. Incomplete submissions will not be considered. When your form is successfully submitted you will receive a confirmation e-mail from surrender@littleguild.org. We will contact you regarding your request within 2-business days of receipt.
**PLEASE NOTE YOU WILL BE REQUIRED TO UPLOAD A PHOTO OF THE DOG AT THE END OF THIS FORM. FORM CANNOT BE SUBMITTED WITHOUT ATTACHING A PHOTO.
